The stretch of Route 236 that holds everything together

Lincolnia runs along Little River Turnpike between Annandale and Alexandria, and it looks like what it is: garden apartment blocks, brick ranch houses from the fifties and sixties, a strip of shops where you can eat in four languages before noon. A lot of the people who keep Northern Virginia running live here. Framers and electricians, hospital techs, drivers, cleaners, restaurant staff, home health aides. Many households hold three generations under one roof and two or three paychecks going into one budget.

Anh "Alice" Tran, PMHNP-BC, FNP-BC, is a dual board-certified psychiatric mental health and family nurse practitioner licensed in Virginia. She treats adults 18 to 65. Visits can be entirely in English or entirely in Tiếng Việt with no interpreter, and the office is close: 3060 Williams Drive, Suite 300, Fairfax, VA 22031, roughly fifteen to twenty minutes west along the same road, with free parking outside the building.

What we hear

What brings Lincolnia adults in

People here rarely arrive saying they are depressed. They arrive saying they cannot sleep, they are angry at everyone, their chest feels tight, or they cannot keep track of anything anymore. Those are the same conversation, just started from a different door.

Bodies that got used up

Twenty years of lifting, kneeling, standing on concrete. When pain becomes permanent, sleep goes, mood follows, and patience runs out at home first. The pain is one problem. What it did to the rest of you is another, and that part responds to treatment.

Two jobs and no margin

A day shift and an evening shift with an hour of driving in between. Sleep gets treated as the flexible item in the budget. After a year or two of that, anxiety and low mood are not surprising, they are almost predictable.

Being the one who translates

Adults who handle every appointment, lease, and school form for parents and in-laws on top of their own life. It is done out of love and it still costs something. That cost has a name and it is worth treating.

Distance from people you love

Parents, siblings, and children who are still overseas, or a family that got separated somewhere along the way. Grief at a distance is harder to finish, and it can look like irritability or numbness rather than sadness.

Attention that costs money

Missed invoices, forgotten inspections, a job lost over paperwork rather than skill. Undiagnosed ADHD in adults shows up in the wallet before it shows up in a doctor's office.

The cost question itself

Worry about what an appointment will cost keeps people out of care for years, which is why rates here are published up front and benefits are checked before the first visit rather than after it.

Services

What you actually get

A full psychiatric evaluation. One hour, not fifteen minutes. Long enough to hear the whole story: when it started, what your work is like, how you sleep, what you drink and how much coffee it takes to get going, what happened in your family, what you have already tried. Alice also takes a real medical history. Because she is board certified as a family nurse practitioner as well, she checks whether a thyroid problem, low iron, sleep apnea, or an interaction with a medication you already take is part of the picture instead of writing a prescription over the top of it. The services page lays out the full scope.

Medication management, explained in plain words. Before anything is prescribed you hear what it is for, how long it usually takes to help, what side effects to watch for, and what happens if it does not work out. One change at a time, so it stays clear what did what. Prescriptions go electronically to whichever pharmacy is easiest, whether that is a counter on Little River Turnpike or one near your job. Controlled medications such as ADHD stimulants follow Virginia's rules, which sometimes means one appointment has to happen in person in Fairfax. There is more on medication management in Virginia.

Supportive therapy at every follow-up. Not just a dose check. Space to talk about the argument that keeps happening at home, the boss who will not schedule you properly, the parent who does not believe in any of this. When something needs structured, longer-term therapy, Alice will tell you and help you find a therapist rather than pretending a thirty minute visit can do that job.

Conditions

What can be treated here

Adults only, ages 18 to 65, across the usual range of adult psychiatric conditions.

Sleep first, usually. Insomnia is where many plans start, because almost nothing else improves while you are running on four hours. Shift work, pain, and worry all wreck sleep in different ways, and they call for different responses.

Anxiety and panic. Anxiety that lives in the chest and stomach, panic attacks that have sent people to the emergency room certain it was a heart attack, and OCD. The GAD-7 screener is free and takes two minutes.

Depression and grief. Depression that still gets up and goes to work, bipolar disorder, and grief that has stayed longer than people expected. Try the PHQ-9 or read about depression treatment in Virginia.

Trauma and ADHD. PTSD and trauma, including things that happened long before Virginia and things nobody in the family talks about. Adult ADHD evaluations, with the self-check and ADHD treatment in Virginia as starting points.

New parents. Postpartum and perinatal mood changes, plus burnout that has moved past ordinary tiredness.

Video or in person

Whichever one you will actually keep

Lincolnia is close enough that in-person visits are realistic. The office is at 3060 Williams Drive, Suite 300 in Fairfax, about fifteen to twenty minutes west, parking is free, and there is no garage ticket to sort out afterward.

Video works too and costs you no drive at all. Appointments open at doxy.me/alicetran in any browser, no app to download and no account to make. You need to be physically somewhere in Virginia while the visit happens. Every visit is by video rather than by telephone, so if the picture drops you rejoin the same link, and the office will call you to help get it working again so the appointment finishes on video. More on how this works is on the Northern Virginia telehealth page.

Your first visit

What happens the first time

Forms come to you ahead of time. Fill in what you can. If a question does not make sense, leave it and ask during the visit. Your insurance is checked beforehand so you know the number before you sit down, not four weeks later in an envelope.

The appointment itself is a conversation, not a test. Alice usually starts by asking why now, since the reason something became unbearable this month rather than last year is often the most useful fact in the room. She works through symptoms, sleep, work, family history, and anything you are already taking, including anything from a pharmacy back home. Then she tells you what she thinks is going on and what she would suggest, and you decide together. Bring a list of medications if you have one. Most people come back in two to four weeks.

Why this practice

Reasons this may fit

You see the same person every time. Not a different provider each visit, and not whoever happens to be free. What worked for you in March is the reason a particular choice makes sense in September, and that only holds together when one clinician is keeping track.

Alice trained under psychiatrist Dr. Errol Segall, MD, who has over 50 years of clinical experience, and she carries dual board certification in psychiatric mental health and family practice. For people who work physical jobs and already take medication for blood pressure or diabetes, that medical background is practical rather than decorative.

Prices are posted rather than quoted after the fact: $400 for the first sixty minute visit, $200 for a thirty minute follow-up, $250 for a forty-five minute follow-up, with no memberships and no hidden fees. Aetna, Anthem, Cigna, Medicare, and Medicaid are accepted. See rates or read about Alice.

And the language question is settled at the door. Full care in Tiếng Việt, full care in English, or both in the same sentence. Details are on the Vietnamese psychiatric care page.

Questions

Straight answers for Lincolnia

I leave for work at five in the morning. Are there times that fit a shift like that?

Standard hours are Monday through Thursday, 9:00 AM to 4:00 PM Eastern, which is difficult if you are on a job site by six. Evening or weekend appointments may be available on request, so say what your schedule looks like when you call or text (703) 791-9099 and we will see what can be arranged rather than assuming it cannot.

Three generations live in my house. Where am I supposed to take a video visit?

A parked car is the most common answer, and it works fine. Others use a bedroom with headphones, a break room at work, or a quiet corner of a library. The visit opens at doxy.me/alicetran in a browser, so any phone, tablet, or laptop will do. What matters is that you can speak without being overheard and that you are physically in Virginia.

My back has hurt for years and now I cannot sleep or think straight. Which problem do I even bring up?

Bring up all of it. Alice does not treat the pain itself, but the sleep loss, low mood, short temper, and hopelessness that build up around chronic pain are exactly what this practice treats, and they are treatable even when the pain is not going away. She will also want to know what your other doctors have prescribed so nothing conflicts.

I do not have insurance right now. What is the real number I would pay?

The first visit is sixty minutes for $400. After that a thirty minute follow-up is $200 and a forty-five minute follow-up is $250. That is the whole price. There is no membership, no signup charge, and no separate fee for the prescription. If you do have Aetna, Anthem, Cigna, Medicare, or Medicaid, those are accepted and benefits are checked before the first visit.

My mother says medicine for the nerves is for weak people. How do I even start that conversation at home?

You are not the first person in this neighborhood to ask. It often helps to frame it the way you would a blood pressure problem: something in the body is off, there is a treatment, and treating it early keeps it from getting worse. If it would help, a family member can join part of a visit and hear it explained directly, in Vietnamese if that is the language the conversation needs to happen in.

Can the whole appointment be in Vietnamese, including the parts about paperwork and cost?

Yes, all of it. Alice speaks Vietnamese and English, so the evaluation, the medication discussion, the questions about insurance, and the follow-ups can happen entirely in Tiếng Việt with no interpreter in the room. You can also move between the two languages mid-sentence, which is how most bilingual conversations actually run.

I drive a truck for a living. Will being treated affect that?

Tell Alice at the first visit, because it changes what makes sense to prescribe. Some psychiatric medications cause drowsiness, especially in the first couple of weeks, and that is a real consideration for anyone behind a wheel all day. Options are chosen with your work in mind, and any employer or licensing paperwork you need to satisfy is worth raising early rather than after a prescription is written.

Is your office on Little River Turnpike, or do I have to get on the Beltway?

No Beltway needed. The office is at 3060 Williams Drive, Suite 300, Fairfax, VA 22031, in the Merrifield area, and most Lincolnia patients simply follow Little River Turnpike west through Annandale and come up from there. It is usually a fifteen to twenty minute drive, and parking at the building is free.

My English is good enough for work but not for talking about feelings. Is that a problem?

It is extremely common and it is not a problem here. Technical vocabulary and emotional vocabulary are learned in different places, and plenty of people who negotiate contracts in English cannot describe grief in it. Use whichever language the feeling actually lives in. Switching languages partway through a sentence is welcome.

Does anything get reported to my employer or to any government agency?

No. This is a private practice and your record is protected under HIPAA. Nothing goes to an employer, a landlord, or any agency without your written permission. You are not asked about immigration status, and it is not needed in order to be evaluated or treated.

What happens if a job runs long and I miss my appointment?

Let the office know as early as you can by calling or texting (703) 791-9099, and it gets rescheduled. Appointments are booked one at a time rather than locked into a standing weekly slot, which suits work that does not repeat the same way every week. The current cancellation policy is posted on the rates page.

What do I do if something happens tonight and I cannot wait for an appointment?

This practice is not a crisis service and messages are not watched overnight. If you or someone in your home is in danger or thinking about suicide, call or text 988 for the Suicide and Crisis Lifeline, or go to the nearest emergency department. Support is available in multiple languages.
