Del Ray is one of the few places in Northern Virginia that genuinely functions like a small town. Mount Vernon Avenue does the work of a main street, people know their neighbors' dogs by name, and the parade turnout says something real about how much residents like living here. That closeness is a resource. It can also make it feel like there is nowhere to go when things are not fine, because half the avenue would recognize you walking into a clinic.

Anh "Alice" Tran, PMHNP-BC, FNP-BC, is a dual board-certified psychiatric mental health and family nurse practitioner licensed in Virginia, treating adults 18 to 65. Del Ray patients are seen almost entirely by video, from a home office, a kitchen table after the school run, or a parked car. Three services, and what each one involves

Psychiatric evaluation. Sixty minutes, and none of it rushed. The conversation covers when symptoms began, what else was happening then, what you have tried and how it went, family history, sleep, caffeine, alcohol, and what a normal weekday actually feels like from where you are standing. Parents in particular arrive with the story already compressed into an apology. The hour exists so it can be uncompressed. If someone labeled you years ago in a fifteen minute appointment, part of the work is checking whether that label was ever right.

Medication management. Nothing gets prescribed without an explanation you can repeat to your partner afterward: what the medication is meant to change, roughly how long that takes, what side effects tend to show up in the first two weeks, and what the plan is if it does not suit you. Adjustments track your response rather than a fixed schedule. Prescriptions go to whichever pharmacy is on your normal route. Controlled medications, stimulants for ADHD included, follow Virginia rules, which can mean one in-person appointment at the Fairfax office.

Supportive therapy. Follow-ups involve actual conversation. That might be untangling resentment over an uneven division of labor at home, deciding whether to step back from a job, or building a realistic sleep plan when a child still wakes at night. When the situation calls for structured longer-term therapy, such as trauma processing or a full CBT course, Alice says so directly and helps you find someone, and she coordinates with a therapist you already have. Two jobs, no bench

Both adults working full time, both commutes non-trivial, and no grandparents nearby. The system holds until one thing goes wrong, and then nothing has slack in it. That is a structural problem people tend to experience as personal failure.

Attention. Adult ADHD, very often diagnosed for the first time by a parent who recognized themselves while reading about their child. The adult ADHD self-check is a reasonable first step.

Trauma, burnout, and sleep. PTSD and trauma responses, burnout that has become clinical, and insomnia that no longer answers to good habits.

Video from home, Fairfax if you want the room

Video is the primary route, and for a neighborhood built around walking and school schedules it is usually the one that survives contact with a real week. A 30 minute follow-up from your own house costs 30 minutes. Visits are at doxy.me/alicetran, secure, and always by video rather than telephone. In-person appointments are available in Fairfax at 3060 Williams Drive, Suite 300, about 25 minutes up the road with free parking on site. A common pattern is an in-person evaluation followed by video for everything after. You are seeing the same provider either way. What the first appointment looks like

Paperwork comes to you in advance so the hour is not spent on forms. Fill in what you know and skip what you do not; there is no grade attached. Coverage is verified beforehand so the cost is settled before the appointment rather than after.

You see Alice at every appointment. There is no rotating panel and no getting handed to whoever has an opening. In psychiatry, continuity is not a nicety. What happened when a dose changed in February is the information that tells you what to do in September, and it only helps if the same person is holding it.

Alice trained under psychiatrist Dr. Errol Segall, MD, who has more than 50 years of clinical experience, and she carries dual board certification in psychiatric mental health and family practice. That second certification matters more than it sounds: postpartum thyroid changes, iron deficiency, sleep apnea, and interactions with medications you already take get considered rather than skipped, which is exactly where a lot of fatigue complaints get mishandled.

Things Del Ray patients ask first

Del Ray is small and people talk. How private is this really?

Video visits mean there is no waiting room on Mount Vernon Avenue where somebody from the block might see you. Your records are confidential health records and are not shared without your written permission, except in the narrow safety situations the law requires. Nobody at your child's school, your employer, or your neighborhood association learns anything from this practice.

I have a toddler at home. What happens if my kid walks in during a video visit?

It happens constantly and it is fine. Alice sees a lot of parents of small children, and an interrupted appointment is still a useful appointment. If you need a moment, take it. If the timing is truly impossible, we can move the visit to a window when there is a second adult in the house.

Can postpartum depression start more than a year after the birth?

Mood changes connected to the postpartum period do not always follow the timeline people expect, and they can surface well after the newborn stage, particularly around weaning, a return to work, or a second pregnancy. It is worth an evaluation rather than an assumption that you missed the window.

Can my spouse and I both be patients here?

Yes, as separate patients with separate records. Alice does not do couples therapy, and what one of you says stays with that record unless you give written permission otherwise. Some two-career households in Del Ray find it simpler to have one provider who understands the shared logistics. Others prefer to keep it entirely separate, which is equally fine.

Are there any appointment times outside the standard workday?

People keep telling me my anxiety is just what having young kids feels like. How would an evaluation be different?

An evaluation separates ordinary exhaustion from something that has a shape and a course of its own. Alice looks at when it started, whether it lifts on a good weekend, what your sleep does when the children sleep, and whether the worry attaches to things that are not actually about the kids. Sometimes the answer is that you are stretched thin. Sometimes it is a treatable condition sitting underneath being stretched thin.

What ages does this practice treat?

Adults 18 to 65 only. Alice does not see children or adolescents, and she does not take new patients over 65. If your teenager needs psychiatric care, ask your pediatrician for a child and adolescent referral.

Do I need my family doctor to send a referral first?

No referral is needed. You can book directly online, or call or text and we will get you scheduled. If you would like your primary care office kept in the loop afterward, that is easy to arrange with your written permission.

What actually happens at the second appointment?

The second visit is where the plan gets tested against reality. You report what you noticed, including the parts that were unpleasant, and Alice adjusts. Doses go up, down, or stay put based on your response. It is normal for the first plan to need tuning, and that is not a sign that something went wrong.

My English is fine until I am upset. Can we switch to Tiếng Việt mid-conversation?

Yes. Care is available fully in English, fully in Tiếng Việt, or moving between the two as you go, with no interpreter involved. For a lot of bilingual patients, the language that holds the feeling is not the language that holds the work vocabulary, and there is no reason to fight that during an appointment.

Is medication the only thing on offer here?

No. The practice provides psychiatric evaluation, medication management, and supportive therapy, and not every plan includes a prescription. Some patients come for an evaluation, get a clear explanation of what is happening, and leave with a plan built on sleep, structure, and a referral to a therapist.
