The visit opens as a conversation, not a checklist. Alice usually asks why now, because what tipped the decision after months or years of consideration is often the most useful sentence of the hour. From there the discussion works through symptoms, personal history, medical background, family history, and anything you are currently taking, including supplements. At the end she says what she is thinking and why, lays out the options with their trade-offs, and you decide together. Most people leave with a follow-up two to four weeks out.

The three things this practice does

Psychiatric evaluation. Sixty minutes, unhurried on purpose. Symptoms are only part of it. The rest is context: what your week is genuinely shaped like, who depends on you, what you have already tried and how it went, what your sleep looks like on a Sunday night versus a Thursday, and what medical conditions run in your family. People who have been managing without help for a long time often find this is the first time anyone has asked for the whole picture in one sitting.

Medication management. When medication is part of a plan, the reasoning is explained first: what it is for, how long a real response usually takes, which side effects matter in the first two weeks, and what happens if it turns out to be wrong for you. Adjustments follow your response. Prescriptions go electronically to whichever pharmacy suits your errands, whether that is on Ashburn Village Boulevard or nearer the hospital. Controlled medications, including stimulants, follow Virginia rules, which may require an in-person visit in Fairfax.

Supportive therapy. Follow-ups make room for the actual situation, not just a medication check. That might be setting a boundary with a family member, deciding how much to tell your children, or figuring out how to keep a treatment plan alive during a stretch when you are the one holding everything up. Alice trained under psychiatrist Dr. Errol Segall, MD, whose clinical experience runs past 50 years, and she is board-certified in both psychiatric mental health and family practice. That combination is unusually useful in a neighborhood full of people in their forties and fifties, because thyroid function, anemia, sleep apnea, perimenopausal changes, and interactions with blood pressure or diabetes medications are considered rather than deferred.

Costs are posted and flat: $400 for the 60 minute initial evaluation, $200 for a 30 minute follow-up, and $250 for a 45 minute follow-up, with no memberships and nothing tacked on. Is there an office in Broadlands or near Inova Loudoun Hospital?

No. The practice has one office, at 3060 Williams Drive, Suite 300, Fairfax, VA 22031, in the Merrifield area, with free on-site parking. Broadlands is served by video, which for Loudoun is the honest and workable route. In-person appointments in Fairfax remain available if you prefer them.

My children are at school all day. Can I be seen mid-morning?

Mid-morning is usually the easiest time to get. Appointments run Monday through Thursday, 9:00 AM to 4:00 PM Eastern, and the slots between drop-off and pickup are exactly the window most Broadlands parents book. Evening and weekend times may be available on request.

I am looking after my own household and an aging parent at the same time. Does that belong in a psychiatric visit?

It absolutely belongs there. Caregiving load is one of the most reliable drivers of exhaustion, resentment, disrupted sleep, and low mood in this age group, and it is often the thing nobody has asked about. It gets treated as clinical information, not as a personal failing.

I work in healthcare locally. Will being a patient here affect my license or my employer?

Getting psychiatric care is not itself reportable to a licensing board, and nothing is disclosed to your employer without your written authorization. Clinicians, nurses, and first responders are seen routinely in this practice. If you have a specific concern about a credentialing question, raise it at the first visit and it can be discussed directly.

How do refills work between appointments?

Prescriptions are written to carry you to the next scheduled visit, so the usual answer is to keep follow-ups on the calendar. What happens if the video freezes partway through my appointment?

You rejoin the same link at doxy.me/alicetran, and the office will call to help you get reconnected so the visit finishes on video. Visits are not switched to a telephone appointment. It happens occasionally and it is not a problem worth worrying about in advance.

Do you use rating scales, or is the visit all conversation?

Both. Brief standardized measures can help track change over time, and the free self-checks on this site for ADHD, anxiety, and depression are a reasonable way to organize your thoughts beforehand. They inform the conversation rather than replacing it. No score decides anything on its own.

I tried an antidepressant years ago and hated how it felt. Is that a dead end?

No. A bad experience with one medication is useful information, not a verdict. What matters is which one, at what dose, for how long, and what specifically felt wrong. That detail often points toward a different option or a different approach entirely, including plans that do not center on medication.

Is Medicare accepted?

Yes. Medicare, Medicaid, Aetna, Anthem, and Cigna are accepted, with your plan verified before the first appointment. Self-pay rates are $400 for the 60 minute initial evaluation, $200 for a 30 minute follow-up, and $250 for a 45 minute follow-up, with no memberships and no hidden fees.
