Where this starts

Almost everyone here arrived recently

Brambleton is one of the newest places in a county that keeps building. Townhomes go up in tight rows, single-family streets fill in behind them, and Brambleton Town Center anchors a community whose median age skews strikingly young. Walk the sidewalks on a Saturday morning and it is strollers, dogs, and construction fencing. Most households moved in within the last few years, many from another state or another country, and almost none of them have a grandparent twenty minutes away.

Anh "Alice" Tran, PMHNP-BC, FNP-BC, is a dual board-certified psychiatric mental health nurse practitioner and family nurse practitioner, licensed in Virginia and treating adults 18 to 65. There is no office in Brambleton or anywhere in Loudoun; care here happens by video. For a household with small children and no backup, that is not a compromise, it is the only version of this that gets scheduled and kept.

The shape of it

What tends to bring Brambleton adults here

The demographics of this community produce a distinctive set of pressures. Two incomes, two long commutes, very young children, and no family nearby is a combination that runs people down predictably. Loudoun's large and growing South Asian and East Asian communities are heavily represented here, and for many families that means parents and in-laws are on another continent rather than down the road.

The first year after a birth

Postpartum and perinatal mood changes are common, treatable, and frequently missed because the checkup is brief and the honest answer is hard to say out loud. Anxiety in this period often presents as constant scanning for danger rather than sadness.

No one to hand the baby to

When both sets of grandparents are thousands of miles away, there is no such thing as an hour off. The absence of relief is a clinical variable, not a lifestyle detail, and it changes what a workable plan looks like.

Two commutes, one household

Partners leaving before dawn in opposite directions and returning after bedtime. The logistics work on paper and leave nothing for each other. Resentment and flatness build slowly in exactly this arrangement.

Friendly street, no close friends

Waving at neighbors is not the same as having someone to call at nine on a bad night. In a neighborhood where nearly everyone is new, connection takes far longer to form than people expect, and the gap is genuinely painful.

The identity you set down

A career paused for childcare or a move, and a version of yourself that has gone quiet. Grief for a former life is real grief, and it does not require anyone to have died.

Attention that unravels under load

Adult ADHD often becomes undeniable when a household of small children multiplies the number of things to track. Many people are evaluated for the first time in exactly this stretch of life.

The first hour

What actually happens when you book

Paperwork comes to you first, so the appointment is spent talking rather than typing. Insurance is verified beforehand, which means the number you owe is known in advance. When the time comes you open doxy.me/alicetran in a browser, a few minutes early, from anywhere private in the house.

Alice generally opens with why now. In this neighborhood the answer is often a specific moment: a morning that fell apart, a comment from a partner, a night that went badly. From there the hour covers symptoms, personal and family history, pregnancy and postpartum history where relevant, medical background, and everything you currently take. She then explains what she thinks is going on and what the options are, including the trade-offs of each, and you decide together. A child in the background is fine and does not need apologizing for. Most people schedule a follow-up two to four weeks later.

Services

What is available

Psychiatric evaluation. A full 60 minutes, which is the amount of time it takes to get past the summary and into the specifics. For people who recently arrived in Virginia, this includes care received elsewhere, medications tried in another state or country, and family history that has never been written down anywhere. For anyone in the perinatal period, it includes the pregnancy, the birth, feeding, and sleep, because those details change what treatment is appropriate.

Medication management. Before anything is prescribed, you hear what it is for, how long a real response usually takes, which side effects matter early, and what the alternative is if it does not suit you. Pregnancy and breastfeeding considerations are discussed openly rather than avoided, so decisions get made with the actual information. Prescriptions go electronically to whichever pharmacy fits your route. Controlled medications, including stimulants, follow Virginia requirements, which can mean an in-person appointment in Fairfax.

Supportive therapy. Follow-ups are conversations, not five-minute refill checks. They might cover how to divide nights with a partner, how to talk about a diagnosis with family who are far away and worried, or how to protect one hour a week that belongs to you. When someone needs structured therapy, such as trauma-focused work or a full CBT course, Alice says so and helps arrange it, and she coordinates with therapists already involved. The services page has more.

How visits happen

Video, honestly, because that is what works out here

Brambleton is a long way from any adult psychiatric office, and pretending otherwise helps nobody. Care is delivered by video at doxy.me/alicetran, which opens in a browser with no app to install and no account to create. Every visit is by video; there is no telephone version of an appointment. If the connection fails partway, you rejoin the same link and the office calls to help you get back on so the visit finishes as it began. You must be physically located in Virginia during the appointment.

In-person visits are available at 3060 Williams Drive, Suite 300, Fairfax, VA 22031, in the Merrifield area, with free on-site parking. It is roughly a 45 to 60 minute drive depending on the Greenway and the time of day. Some people do an initial evaluation in person and everything else by video. The Northern Virginia telehealth page explains the mechanics in more detail.

Why here

Reasons this setup suits Brambleton households

One clinician, every visit. Repeating your story to a new face each time is exhausting under normal circumstances and close to impossible when you are already running on four hours of sleep. Continuity means the history is already known and the conversation starts further along.

Alice trained under psychiatrist Dr. Errol Segall, MD, whose clinical career spans more than 50 years, and she holds board certification in both psychiatric mental health and family practice. The family practice training is directly relevant here: thyroid changes after pregnancy, iron deficiency, and interactions with medications you are already taking get considered rather than assumed irrelevant, which matters when the presenting complaint is exhaustion.

Pricing is flat and published: $400 for the 60 minute evaluation, $200 for a 30 minute follow-up, $250 for a 45 minute follow-up, with no membership and no add-ons. Aetna, Anthem, Cigna, Medicare, and Medicaid are accepted, verified before your first visit. See the rates page, or read more about Alice. Care is available fully in English or fully in Tiếng Việt, with no interpreter in the room.

Questions

Things Brambleton patients ask first

Do you see patients at Brambleton Town Center or anywhere in Loudoun in person?

No. There is no Loudoun location. The office is at 3060 Williams Drive, Suite 300, Fairfax, VA 22031, in the Merrifield area, with free on-site parking. Brambleton is served by video, and for a community this far out that is the access route that actually works. Fairfax visits are available if you specifically want one.

I had a baby four months ago. Is that too late or too early to bring up mood changes?

Neither. Postpartum and perinatal mood changes can begin during pregnancy and can surface many months after delivery, and four months in is an entirely ordinary time to seek help. Waiting for it to pass on its own is common and often costs people a year they did not need to lose.

Can I do the visit with a baby or toddler in the room?

Yes. Parents of small children do it constantly, and a visit interrupted by a child is a normal visit. If you would rather have quiet, the office can help you find a slot during a nap or when your partner is home, but nobody needs to arrange childcare in order to be seen.

We moved here last year and barely know anyone. Can a psychiatric visit really help with that?

It can, in two ways. First, isolation after a relocation reliably worsens anxiety and low mood, and naming that accurately changes the treatment plan. Second, part of the work is practical: what a realistic week looks like, where connection could plausibly come from, and what is getting in the way of it.

Our family's first language is not English. How does that affect the visit?

Visits can be conducted fully in English or fully in Tiếng Việt with no interpreter involved, and you can move between the two mid-sentence. Beyond language, cultural expectations about family, marriage, and what counts as a problem worth mentioning are treated as part of the clinical picture rather than something to work around.

My spouse commutes into DC and is never home during your hours. Can they still be a patient?

Often yes, because a video visit does not require being at home. Many commuters join from a parked car, a booked room at the office, or before the drive begins. Standard hours are Monday through Thursday, 9:00 AM to 4:00 PM Eastern, with evening or weekend times sometimes available on request.

How is the 60 minute first visit different from a 30 minute follow-up?

The first visit builds the whole picture: history, symptoms, medical background, family history, sleep, substances, and what you have already tried. That takes an hour and costs $400 self-pay. Follow-ups review how the plan is working and adjust it, running 30 minutes at $200 or 45 minutes at $250 when there is more to cover.

Is a video visit as useful as sitting in an office?

For psychiatric evaluation and medication management, video works well, and it removes the barrier that stops most Loudoun parents from ever starting. There are situations where being in the room is better, and certain controlled medication requirements under Virginia rules can call for an in-person appointment. Alice will tell you if that applies to you.

I cannot tell whether this is burnout or depression. Where do I even start?

Start by booking the evaluation and describing it exactly as you just did. Sorting that out is a clinical question, not something you should have to answer before you arrive. The two overlap, they respond to different things, and the distinction usually becomes clear once the whole timeline is on the table.

I am exhausted all day and then wide awake once the house is finally quiet. What is that?

It is one of the most common patterns described by parents of young children, and it usually has more than one cause: delayed wind-down, anxiety that only gets airtime after bedtime, and a body clock pushed later by months of interrupted nights. It is treatable, and it is often where a plan starts because sleep improves everything else.
