Humana's health plans in Virginia are Medicare Advantage plans: PPO, Regional PPO, HMO, HMO-POS, Private Fee-for-Service, and Special Needs Plans for people who have both Medicare and Medicaid, who live with diabetes or heart disease, or who live in a care facility. All of them cover outpatient mental health care, including psychiatric evaluations and medication management visits with a board-certified psychiatric nurse practitioner.
What you pay is usually a fixed copay for a specialist or mental health visit, and it differs from plan to plan. HMO plans may ask you to choose a primary care provider and may require a referral for specialists; PPO plans generally do not. Before your first appointment, call the member services number on the back of your card, confirm your outpatient mental health benefit, and ask whether a referral is needed. This practice also verifies your eligibility before your visit.

In Virginia, yes. Humana no longer sells employer or individual commercial medical plans, so a Humana health card today is a Medicare Advantage plan, a Medicare Special Needs Plan, or a group Medicare plan through a former employer. If your card says Humana Military, that is TRICARE, which is handled separately.
It depends on your plan type. Humana PPO and PFFS plans generally let you see a specialist without a referral. Humana HMO plans may require one from your primary care provider. Call the number on your card to confirm.
Yes. Humana Dual Select and Humana Gold Plus SNP-DE are Dual Special Needs Plans for people with both Medicare and Medicaid, and both are in network. Your cost share on these plans is usually very low.
