Premenstrual dysphoric disorder is a severe mood disorder tied to the menstrual cycle. In the one to two weeks before your period (the luteal phase), symptoms arrive: intense irritability or rage, depressed mood or hopelessness, anxiety, mood swings, fatigue, and physical symptoms. Then, within a few days of your period starting, they lift. The cycling is the signature.
PMDD affects an estimated 3 to 8 percent of menstruating women, and it is not a milder problem than depression; during the luteal phase it can be just as severe, including thoughts of self-harm. The cause is not abnormal hormone levels but an abnormal brain sensitivity to normal hormonal shifts, which is why "your labs are normal" never made you feel better.
Most women with PMDD spend years being told it is just PMS, just stress, or just their personality. The average delay to diagnosis is long, and many are treated for depression or anxiety without anyone ever asking them to track symptoms against their cycle, the single most important diagnostic step.
Diagnosis starts with tracking: two months of daily symptom ratings against your cycle confirms the pattern and distinguishes PMDD from depression or anxiety that simply worsens premenstrually, which is treated differently. I will give you a simple way to do this.
PMDD responds to treatment. SSRIs work differently and faster in PMDD than in depression, and can be taken continuously or only during the luteal phase.
